I really need to read a first time wincest fic where they get together only weeks before Dean's deal comes due because I am a fucking masochist.

Circa season three, Dean's going to hell, and he wants one thing from Sam.

They're in a motel and Dean's time is ticking down fast. Perhaps it's after he talks to Ruby and she admits there's no saving him.

So he goes back inside and he sits down on the bed opposite Sam. And he's like, I need you to do something for me.

After much angst and tears Sam admits he's wanted it too, thinks he came back wrong, but he gives Dean what he wants. So Dean kisses Sam, then retreats. But Sam has had a taste and wants more, so he kisses Dean more passionately. The dam bursts, and he's hungry for more.

It progresses to feeling up, taking off clothes. Lots of FOMO, this may be their only chance. Sam rims Dean, and they eventually fuck.

Bittersweet (emphasis on the bitter) ending.
