Sam gets deaged. He still tops.

Or they both get deaged. To like 16 and 20. Sam still tops.

Maybe they did it to themselves. Like things are starting to get creaky, knees aren't working right etc. So they try this fountain of youth spell, so they can keep fighting.

But it goes awry. Sam ends up 14 and Dean's like 18. Sam still tops.

Or Dean does it to Sam. With his big body things are crapping out sooner than they should, despite his clean living. Dean does a youth spell on him and Sam ends up like 16. Or 14. If we're going to hell anyway...

Or like, the witch Patrick. Something like that, gets Sam. And they attempt to fix it, but go too far back and Sam ends up 16.
