In which Sam is infected by and turned into a dragon, and in which they discover too late that it's how these creatures reproduce--they're infected, and become a dragon (fuck established lore) already full of eggs, which they then, using their new penis-turned-ovipositor, place inside a host during sex.
In established lore, dragons look like people. Sam's bitten or scratched, almost dies from the infection, then recovers and they think everything is good.
But during sex, he knots Dean, and they can't separate. They're helpless to stop Sam depositing up to a dozen eggs inside Dean.
Afterwards they're researching how to get them out of Dean when they discover that Sam will die after the eggs are gestated as by then he will have outlived his purpose.
So their focus shifts to the race to cure Sam before Dean lays the eggs, meaning that by the time they figure out how to save Sam, Dean's about to pop.
They save Sam, and then Sam helps Dean deliver the eggs.

There's Lore. The only way to cure Sam is for him to eat his offspring. So they prepare for it. It's almost clinical to begin with, also Sam's dragon dick is huge and RIDGED on the underside, if Sam takes him from behind its going to blow Dean's mind.
Dean's expects eggs he can't detect, expects to get fucked and done, and really, neither of them know what to expect. And then Sam kind of knots Dean. He can't pull out and Dean can't get away.
